Abstract Body
Introduction: Cydnidae pigmentation is extremely rare due to its under reported tendency and often people used to ignore its course of nature. Cydnidae insects commonly known as burrowbugs usually stay underground and feeding on roots of plants. The pigmentation is caused by the hydrocarbon which is produced by the special glands in the thorax of the adult bugs and abdomen of the nymph. The rash is like brownish globules and pigmentation around sweatglands in the dermoscopy Case: A 12-year-old girl presented with unknown bite to right side sole, who developed multipleecchymosis like non blanching rash on the sole, after one hour. The rashes were non itchy , non-tender and only involving the right sole and not disappear even after washing with soap and water. All examination findings are unremarkable except the rash. Her bloods were normal. Child was started on IV co-amoxiclav and antihistamines prescribed for the mild itching which developed later. Child also had a previous history of same kind of rash, in the face ,6month back. A diagnosis of Cydnidae pigmentation was made based upon the history and the nature of the rash. Conclusion: Asymptomatic atypical pigmentation may be secondary to many causes. But aclear history and examination guide us to the correct diagnosis. Though the Cydnidae pigmentation is very rare in Sri Lankan literature, always consider it as a cause when a child present with unknown bite with an atypical rash.
