CHANGES IN BODY WEIGHT AND NUTRITIONAL STATUS OF CHILDREN WITH CONGENITAL HEART DISEASE AFTER TRANSCATHETER CLOSURE OF HEART DEFECT

Background : Congenital heart disease (CHD) is the most common congenital disorder in children, where there is a macroscopic structural abnormality of the heart or large intrathoracic vein. Children with CHD are at risk of malnutrition. Previous study demonstrated a significant improvement of nutritional status 3-6 months after cardiac surgery and progressively improved with increasing time. This study aims to determine changes in body weight and nutritional status in pediatric patient with CHD before and four months after transcatheter closure of heart defect. Material : We conducted a retrospective study, which identified children with CHD who underwent transcatheter closure in Dr. Sardjito General Hospital from July 2013 to January 2018. Changes in body weight and nutritional status before and after transcatheter closure of heart defect were calculated. Results : A total of 25 patients (13 males and 12 females) were identified with median age was 3.75 years (interquartile range 3 years to 4 years and 10 months, ranged 4 months to 13 years). The percentage of CHD types ie; ventricle septal defect, patent ductus arteriosus, and atrial septal defect were 52%, 28% and 7% respectively. Compared to baseline at the time of surgery, weight improved 4.27% and 6.65% at one and four months after transcatheter closure respectively. The improvements of weight-for-age Z score were 0.326 and 0.739 respectively. However, height-for-age Z score decreased 0.069 SD one months after surgery and increased 0.066 SD four months after surgery. Conclusions : Body weight, weight-for-age Z score, and weight-for-height increase at one month and four months after transcatheter closure of heart defect, but not for height-for-age.
