RISK OF IRON DEFICIENCY IN 4-6 MONTHS OLD INFANTS RELATED TO EXCLUSIVE BREASTFEEDING

Background : Iron Deficiency Anemia (IDA) is the most common type of anemia that has strong impact on the growth and development of children. Due to increased iron requirements for the growth, Children, especially infants are sensitive to iron deficiency (ID). Milk is the only sources of iron for infants under the age of 6 months, including breast milk, cow's and infant formula. Many previous studies showed decreasing incidence of IDA on exclusively breastfed infants compared to cow's milk or infant formula.
Material : A total of 164 subjects were recruited consecutively in Gianyar Hospital, Permata Hati Hospital and 6 health centers throughout the Klungkung’s district, on period June to November 2016, an analytic observational study with cross-sectional design. The iron status was checked (complete blood count, SI, TIBC, ferritin) on all subjects and then they have to complete the questionnaire. The data distribution was normalized with the Kolmogorov-Smirnov test. Bivariate analysis using Chi-square test and multivariate logistic regression analysis on factors related to the bivariate analysis. P <0.05 was significant.
Results : There are 110 subjects consist of 55 infants exclusively breastfed and 55 infants with formula. >From 110 subjects there are 11 infants with ID (2 infants in exclusively breastfed group and 9 infants formula group), there was only 2 infants with IDA on formula group. Bivariate analysis of exclusive breastfeeding showed lower risk of ID by 78% (PR=0.22 CI 95% 0.05 to 0.98). Multivariate analysis found significant variables on exclusive breastfeeding group OR 0.12 (95% CI 0.02 to 0.66; p=0.02).
Conclusions : Exclusive breastfeeding can reduce the risk of ID by 88% in infants aged 4-6 months.
